Hospital Israelita Albert Einstein (HIAE) is a patient-oriented institution. Safety, the patient’s right

Patient safety was elected as HIAE’s top priority in 2007. Measures were taken to prevent, eliminate or minimize risks and flaws which might affect our patients. Our Patient Safety Risk Management System is guided by the Institute of Medicine’s (IOM) principles which serve as a reference to Einstein’s Quality System.

This is not new to the Institution. HIAE uses a wide range of processes and monitoring mechanisms to track health care indicators related to patient safety.

Important initiatives were taken in 2007 in respect to patient safety, among them:

  • Creation of a new system which favors and stimulates the notification of adverse events.
  • Investigation of all critical incidents in all areas.
  • Central analysis of all incidents to identify systemic problems.
  • Monthly report on patient safety.
  • Regular meetings with the SBIBAE’s leaders to take preventive/corrective measures.
  • Election of priority areas for risk management.
  • Patient Safety International Symposium with 600 participants from Brazil and Chile.

Further action was taken regarding people, in an attempt to promote a safety culture among associates who are directly and indirectly involved with assistance. The “Safe Attitude” campaign was launched with speeches, fliers, hot site and other resources directed at physicians, support and interdisciplinary teams, corporate associates, patients and visitors. Every month, the campaign, which is still active in 2008, approaches a different subject related to patient safety.

Priority areas in 2007 for patient safety

  • Patient identification.
  • Communication (effective information exchange among professionals, departments and areas).
  • High-risk medication (drugs with similar names and packaging, low therapeutic index, high concentration and other aspects which might cause mistakes).
  • Identification in the Surgical Center (correct patient identification - correct surgery in the correct location).
  • Fall prevention.
  • Review of death causes (focusing on eliminating unexpected deaths).
  • Critical adverse event analysis.
  • Prevention of pressure ulcers.
  • Prevention and reduction of blood infections associated to central venous catheters.
  • Prevention and reduction of pneumonia associated to Adult UCI ventilation systems.
  • Reduction of infections (promotion of correct hand washing).
  • Phlebitis prevention (peripheral venous access).
  • Adequate use of phonation valves.
  • Enteral and Parenteral nutrition.
